About the Role The Behavioral Health Therapist at St. Francis Physician Services provides outpatient mental health services to individuals and families, coordinating with medical and psychiatric teams to integrate behavioral and physical health care. The role utilizes the Collaborative Care Model to support recovery, screenings, counseling, and development of care plans. What You'll Do
- Support mental and physical health care of patients on assigned caseload, coordinating with the patient’s medical provider and meeting weekly with the consulting psychiatrist to review progress.
- Communicate psychiatric recommendations back to the primary provider.
- Screen and assess patients using PHQ-9, GAD-7, and other validated objective rating scales.
- Complete all clinical documentation (treatment plans, assessments, progress notes) accurately and in a timely manner.
- Provide psychoeducation and direct advice regarding symptoms of depression, anxiety, and alcohol misuse; develop and communicate treatment plans to the patient’s primary care provider.
- Utilize data from situational analysis, functional assessment, clinical review criteria and the EMR (EPIC/ConnectCare®) to identify and prioritize patient needs.
- Facilitate multidisciplinary plan of care meetings to maintain rapport and monitor the plan; adjust as required.
- Demonstrate leadership by initiating referrals to community agencies (Adult/Child Protective Services, Med Assist, American Cancer Society, DSS, etc.).
- Actively participate in education, case management care coordination, and psychosocial referral and follow-up as core components of service.
- Assist in preparation of behavioral health problem reports, progress notes, treatment plans, summaries and psycho-socials.
- Perform duties independently; seek guidance as necessary and report to supervisor with suggestions for policy or procedure improvements.
- Maintain strict confidentiality of patient information in accordance with federal law.
- Attend Monthly Team Meetings and Quarterly Multidisciplinary Team meetings.
- Demonstrate knowledge of growth and development across the lifespan and the ability to interpret data needed to address patient needs.
- Facilitate approved support groups once per month or as determined. What We're Looking For
- Required Education: Graduate Degree (Masters) in Social Work, Counseling, Psychology, or related field.
- Licensure/Certification Required: Active South Carolina Licensure (SC State Board of Social Work); Master’s degree in professional counseling or social work; Licensure as an LISW-CP is required.
- Minimum Experience: 3 years of experience in counseling in a medical setting/case management.
- Other Knowledge, Skills, and Abilities: Knowledge of DSM-5 nomenclature, psychotherapy, psychodynamics, therapeutic techniques, interviewing skills, addictions, family and group theories; awareness of age-specific and cultural needs.
