Job title: Behavioral Health Clinician (Family Health Center)
About the Role Responsible for the clinical assessment and outpatient treatment for patients seeking mental health treatment. Serves as a member of an interdisciplinary treatment team.
What You'll Do
- Conducts bio-psycho-social mental health assessments of patients and develops patient-centered treatment plans.
- Administers direct patient care.
- Participates in education and training of Behavioral Health Interns and Family Medicine Residents.
- Documents care provided according to Medicare and other compliance standards, and CHMG and practice standards.
- Monitors schedule in order to maximize patient access.
What We're Looking For
- Education: Five to six years of formal training or education beyond the high school level (e.g., Masters Degree or five-year program in a mental health discipline).
- Certification, Registration & Licensure: Licensure required to obtain: Must be license eligible. Licensed by the state of New Hampshire to perform mental health practice within two years of hire date.
- Experience: At least two years practicing mental health counseling preferred.
