Job title: APRN/PA-C - Hospitalist Program
About the Role The Advanced Practitioner for the Hospitalist Program provides inpatient acute care services, including initial encounters and dispositions, daily rounds, consultations, preventive care and patient education. Care is provided under the supervision of the supervising physician.
What You'll Do
- Obtain patient health history, perform physical examination, develop a relevant differential diagnosis, and document encounters appropriately.
- Orders, interprets, and follows up on diagnostic tests.
- Diagnoses acute and chronic medical problems within the scope of practice, or formulates a comprehensive differential diagnosis and takes the necessary steps to obtain a diagnosis, including appropriate specialty consultations.
- Prescribes and manages medications within the practice standards.
- Provides individualized patient education related to preventive care according to guidelines and relative to the care of the hospitalized patient.
- The Advanced Practitioner documents in the medical record according to the department standards.
- Provides effective communication with the multidisciplinary members of the patients care team, including consultants and specialists.
- Performs medication reconciliation upon admission and discharge to ensure medication safety, eliminate interactions, and avoid omissions upon discharge.
What We're Looking For
- Education: Completion of an Accredited Physician Assistant Program or Advanced Practice Nursing Program.
- Licensure/Certification: Current license to practice in the state of New Hampshire; Current DEA License; Certified by the National Commission on Certification of Physician Assistants, American Academy of Nurse Practitioners, or American Nurses Credentialing Center.
- Certification: ACLS certification.
- Experience: Provider experience required.
