About the Role The Advanced Practitioner provides acute and chronic care services to patients in the inpatient setting, including initial encounters and dispositions, daily patient rounding, consultations, preventive care and patient education. Patient care is provided under the supervision of the supervising physician. We are looking for weekend coverage only (day and/or nights). What You'll Do
- Obtain patient health history, perform physical examination, develop a relevant differential diagnosis, and document encounters appropriately.
- Order, interprete, and follow up on diagnostic tests.
- Diagnose acute and chronic medical problems within the scope of practice, or formulate a comprehensive differential diagnosis and take the necessary steps to obtain a diagnosis, including appropriate specialty consultations.
- Prescribe and manage medications within the practice standards.
- Provide individualized patient education related to preventive care according to guidelines and relative to the care of the hospitalized patient.
- Document in the medical record according to the department standards.
- Provide effective communication with the multidisciplinary members of the patients care team, including consultants and specialists.
- Perform medication reconciliation upon admission and discharge to ensure medication safety, eliminate interactions, and avoid omissions upon discharge. What We're Looking For
- Completion of an Advanced Practice Nursing Program.
- Current license to practice in the state of New Hampshire.
- Current DEA License.
- Certification by the American Academy of Nurse Practitioners, or American Nurses Credentialing Center.
- Current ACLS certification.
- Critical Care/Inpatient experience required.
- Weekend coverage only (day and/or nights). Compensation & Benefits
