About this role

About the Role The Department of Family Medicine at the University of Cincinnati College of Medicine is recruiting Advanced Practice Providers to join the Timothy Freeman, MD, Center for Developmental Disabilities within the UC Health system. The Freeman Center aims to be a national leader in providing specialized, coordinated, patient-centered care to adults with developmental disabilities. The role focuses on delivering primary care to adults with IDD with a 60% community/home visit component and 40% on-site work at the Freeman Center.

What You'll Do

  • Provide primary care to adults with intellectual and developmental disabilities including autism, cerebral palsy, Down syndrome, and rarer syndromes, with 60% of clinical time in the community/home visit setting and 40% on-site at the Freeman Center.
  • Collaborate with an interdisciplinary team.
  • Communicate effectively with patients with varied communication styles, caregivers, families, support staff, and multiple specialists to coordinate care.
  • Document findings, plans, and actions taken according to departmental guidelines.
  • Maintain knowledge of relevant medical/legal issues impacting patient care and maintain records/statistics per department policies.
  • Attend and participate in staff, committee, department, and other administrative meetings.
  • Participate in the department's continuous quality improvement efforts.
  • Provide education to other health care professionals.
  • Perform other duties as assigned.

What We're Looking For

  • Required Education: Master’s Degree in Nursing from an accredited nursing program.
  • Required Trainings/Certifications: Active Ohio APRN license with prescriptive authority; Active Board Certification in appropriate nursing specialty.
  • Required Experience: Three (3) years nursing experience; additional related work experience may be required depending on specialty/sub-specialty.
  • Physical Requirements/Work Environment: Sitting often; repetitive hand motion (typing) often; hearing/listening often; talking often; standing often; walking often; bending seldom; stooping seldom; kneeling/squatting seldom; reaching overhead seldom; pulling/pushing seldom.

  • Preferred experience working with adults with intellectual and developmental disabilities (IDD).
