About the Role Join Endeavor Health's Primary Care Department as an Advanced Practice Provider (APN/PA) in the northwest suburbs of Chicago. Under the direction of collaborating physicians, you will provide care to patients in clinic and hospital settings, performing comprehensive assessments and guiding treatment to promote wellness and manage acute and chronic conditions.

What You'll Do

• Provide care to patients under the direction of the collaborating physician(s).
• Obtain detailed history and perform physical examinations; order imaging, lab tests and diagnostic procedures and interpret results.
• Instruct patients on prescribed therapeutic regimens and pre/post operative instructions.
• Maintain records and write progress notes on status, treatments and procedures.
• Follow patients through their course of treatment in the clinic and hospital setting.
• Collaborate with physicians and the health care team to achieve optimal patient outcomes.
• Prescribe pharmacologic and non-pharmacologic therapies as appropriate.
• Demonstrate evidence-based practice and participate in the development, implementation, and evaluation of standards of practice.
• Assess the learning needs of patients and families; employ strategies, interventions and teaching to promote health.
• Provide comprehensive health assessments, differential diagnoses, and prescribe treatments under protocol in the direct management of acute and chronic illness and disease.
• Promote wellness and prevention in various settings for individuals, families, and communities.
• Work autonomously and in interdisciplinary teams as resources and consultants; contribute to safety outcomes and high-quality patient experience.
• Participate in the development, implementation, and evaluation of service line goals and support the organization's strategic plan.
• Adhere to Endeavor Health policies and procedures.

What We're Looking For

• Licensure: Advanced Practice Licensure (APN) or Physician Assistant (PA) licensure in the State of IL; Federal DEA and Illinois Controlled Substance License required.
• Education: Master’s level program in specialty area.
• Certification: BLS/CPR certification required.
• Experience: Minimum of 1 year APN/PA experience in Primary Care required; 2 years preferred. For APNs, Family Nurse Practitioners only.
• Preferred Qualifications: Experience working in an outpatient family medicine setting; experience with both acute and chronic condition management and preventative care.

Compensation & Benefits

• Salary: $56.44 - $70.58 per hour; pay rate determined by expertise and years of experience.
