Job title: Advanced Practice Nurse (PRN) – Hospice - Southwest NJ
About the Role The Advanced Practice Nurse provides hospice care as part of an interdisciplinary team, conducting recertification visits, communicating findings to the team, and supporting care models. This PRN role requires collaboration with physicians, care teams, patients, and caregivers to contribute to the plan of care and ensure high-quality palliative services.
What You'll Do
- Conduct re-certification visits to hospice patients in accordance with local, state, and federal regulations.
- Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director.
- Facilitate access to appropriate support models of care (hospice, rehabilitation, ongoing aggressive treatment).
- Consult with referring physicians, the acute care team, and other IDT members to contribute to the patient's plan of care.
- Prepare and maintain accurate patient records, charts, and documents.
- Assist in development of clinical practice guidelines/standards to support quality palliative care as needed.
- Ensure collaboration with patients, caregivers, medical directors, and IDT members.
What We're Looking For
- Master’s Degree in Nursing.
- Nurse Practitioner license (in the state of practice).
- 2-4 years of experience in hospice or a related field.
- Must have a valid driver’s license and reliable transportation; willing to travel.
- Demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
- Basic technology skills, including operation of a mobile device.
- Must be able to work in an interdisciplinary team and maintain accurate documentation.
