About the Role This posting is for individuals interested in enrolling in the Adult Day Habilitation Program for adults 21+ with intellectual and developmental disabilities. It explains how to express interest, what to expect, and the intake process. What You'll Do
- Express interest in the Adult Day Habilitation Program by phone at 732-922-0228 (ask for the Adult Program) or by submitting the contact form on the page.
- After contacting the Adult Program and meeting enrollment criteria, provide an up-to-date ISP and, if you graduated high school within the past year, a recent IEP.
- Take a tour of the ten-acre campus; if there is interest in the Program, an individualized intake can be scheduled.
- Schedule an individualized intake with the Program Director and provide required contact information as part of the process. What We're Looking For
- Individuals who are 21 years of age or older with intellectual and developmental disabilities.
- Willingness to engage in the enrollment process and meet criteria for enrollment.
- Ability to provide the requested documentation (ISP, and if applicable, IEP) and to communicate via the provided contact methods.
