ROLE OVERVIEW
You will:
- Be the first point of contact for patients

- Track chronic conditions (BP, sugar, medication adherence)
WHO WE ARE LOOKING FOR
A nurse who:
- Is registered with NMCN with at least 2 years post registration experience
- Has hospital or community health experience
- Communicates clearly and calmly with patients
