About this role
Corewell Health (Spectrum Health) is hiring a Social Worker (MSW) for Behavioral Health at HDVCH in Grand Rapids, MI. Candidates must hold an LLMSW or LMSW license.
You will work collaboratively with an interdisciplinary team to assist patients and families with complex psychosocial needs, including alternate site of care coordination, discharge planning, addressing all forms of identified abuse or neglect, adoption, substance use disorders, end-of-life decision support, referral resources, bereavement support, adjustment counseling, guardianship facilitation, and behavioral health concerns including psychiatric transfers.
Responsibilities include conducting psychosocial assessments with patients and/or families to identify needs and execute a care plan, and communicating with alternate sites of care and community agencies to coordinate transitions and support services.
