About this role
Aledade PBC is hiring a Care Manager to support its Primary Care Program, working closely with primary care practices in Maryland and D.C. (base locations Bethesda, MD and Dover, DE; remote, up to 20% travel). The Care Manager collaborates with each practice's care team and Aledade's interdisciplinary staff to provide telephone-based health coaching, quality improvement, and care coordination for Medicare patients, using Aledade's proprietary population health tool to identify and manage high-risk, high-utilizing patients.
