About the Role The Social Worker provides medical social services under the direction of a physician and the Interdisciplinary Group to help understand significant social and emotional factors affecting a patient’s health status, and to support development of coping mechanisms. As a member of the Optum family and part of the post-acute care team, you will help patients, families, and care teams connect resources and coordinate care to improve outcomes. What You'll Do
- Assist the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as-needed basis.
- Contribute as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis.
