- Plans, coordinates, and delivers patient care using the nursing process (assessment, planning, intervention, implementation, evaluation)
- Performs comprehensive patient and family assessments across physical, emotional, psychosocial, and spiritual domains
- Develops, implements, and updates individualized, patient-centered care plans with measurable goals
- Reviews and revises the plan of care, including interventions, medications, and visit frequency
- Assesses changes in patient condition, communicates findings to the physician, and assists in determining level of care and continued eligibility for hospice services
