About the Role Join Holisticare Hospice as a Per Diem RN Case Manager (RNCM) to provide compassionate end-of-life care across Lehigh and Northampton counties. This role focuses on delivering hands-on hospice nursing, coordinating services, and educating families to support comfort and dignity at home and in the community. What You'll Do
- Deliver hands-on hospice nursing to patients and families
- Coordinate interdisciplinary services and on-call triage as needed
- Educate families and caregivers on care plans and symptom management What We're Looking For
- Active and unencumbered RN license
- 1+ year med-surg/acute RN experience
- Current CPR certification
- EMR proficiency
- Valid driver's license and reliable vehicle
- Hospice experience preferred Nice to Have
- Hospice experience (preferred) Compensation & Benefits
- Salary: $40-$45 per hour
