About the Role We are seeking a Registered Nurse Mentor for Sevita's Community Services to provide clinical leadership and direct nursing support to individuals with intellectual and developmental disabilities in community settings. This is a traveling, full-time role with flexible hours that may require working across multiple locations to be accessible to staff and individuals served. The mission is to help people live well and pursue fulfilling lives. What You'll Do
- Conduct assessments, implement health care plans, and respond to changes in condition.
- Communicate with physicians and external medical centers to ensure consistency of essential medical information.
- Provide technical assistance, medical oversight, and staff training to ensure effective delivery of related services.
- Provide input regarding health care services to be included in each individual's service plan.
