About the Role PurposeCare is seeking a Physical Therapist (PT) to provide home-based therapy in the London, OH area. You will deliver skilled visits, assess patient needs, and support recovery in patients' homes as part of our home health services. What You'll Do
- Provide skilled physical therapy visits in patients' homes, addressing the full scope of a patient's needs and progress.
- Assess, plan, and implement therapeutic interventions and document outcomes; coordinate with caregivers and care teams.
- Travel to patient homes as required, maintain quality care with compliant home health practices and OASIS documentation where applicable. What We're Looking For
- Current state Physical Therapist license
- 1 year PT experience preferred (home health/OASIS preferred)
- Ability to lift up to 50 lbs and physically manage home visits
- Valid driver's license and dependable transportation
