About this role
About the Role As the Physical Therapist in Home Health, you will assess patient care needs related to functional status and mobility, endurance and pain, and other medical conditions. Based on assessments, you will help determine a treatment plan, perform interventions, and evaluate patient progress in a home-based setting. What You'll Do
- Evaluate the patient's functional status and physical therapy needs.
- Assist the physician in developing the therapy plan of care.
- Provide services within the scope of practice per state laws and plan of care, coordinating with other health care team members.
- Observe and report patient responses to treatment and any condition changes to supervising nurse/physician.
- Treat patients to relieve pain, develop/restore function, and maintain maximum functioning.
