About the Role Join CenterWell Home Health as an Occupational Therapist to help patients regain strength, mobility, and independence in the comfort of their own homes. This PRN role focuses on rehabilitation and functional improvement through personalized care delivered in home settings.
What You'll Do
- Assess and screen patients' daily living/work-related skills; develop therapeutic retraining programs with measurable objectives.
- Administer and interpret diagnostic and prognostic tests of function to patients in their homes to assist the physician in evaluating function.
- Confer with the patient's physician and rehab team to develop/revise the Plan of Care.
- Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination, range-of-motion and function; teach compensation measures.
