About the Role Become a part of our caring community. As a Home Health Occupational Therapist at CenterWell Home Health, you’ll help patients regain strength, mobility, and independence in the comfort of their homes. You’ll deliver personalized rehabilitation-focused care to improve functional independence and quality of life for clients in Boulder/Longmont and surrounding areas. What You'll Do
- Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
- Administer and interpret diagnostic and prognostic tests of function in patients' homes or other settings to assist the physician in evaluating the patient's level of function.
- Confer with the patient’s physician and other health care team members and participate in development/revision of the Plan of Care.
