About the Role The MAT Registered Nurse/Care Manager at Sun River Health will join a multidisciplinary team to provide nursing care, conduct MAT-related assessments and care planning, and support harm-reduction and crisis management for patients at the Bay Street Staten Island site. The role focuses on patient advocacy, quality of care, and coordination with external agencies. What You'll Do
- Act as point person and primary contact champion for the site location
- Complete MAT intakes by phone or in person
- Complete MAT Patient care plans
- Complete recovery support plans
- Document all encounters
- Complete all necessary consent forms to coordinate care with external agencies
- Complete discharge paperwork in conjunction with the MAT provider; Drafting, scanning etcetera
- Triage presenting issues & provide basic nursing skills in every visit to patients; attend all visits
