Team/Program: Care Coordination Unit – Non-Clinical Care Coordination Team

Schedule: Monday–Friday

Hours: 8:00 a.m.–5:00 p.m. Eastern Time

In-Office Requirements: Recurring meetings every other month and quarterly goal-review meetings

Preferred Experience: Strong call center experience

Leadership Experience: Demonstrated people leadership and team-development skills
