LTSS Service Care Coordinator
About this role
The LTSS Service Care Coordinator at Centene Corporation supports the care management of 28 million members with a focus on home visits for Physically Disabled and Aging Waiver services and occasional visits to members in Supported Living facilities. This hybrid-remote role requires about 50% local travel and prefers candidates who reside in northwestern Illinois, with Peoria County highly preferred. The position is part of the LTSS Case Management – Community team and serves Physical Disability & Aging Waiver Members, typically Monday through Friday, 8 a.m. to 5 p.m. CT with a one-hour lunch.
