About the Role As part of Bluegrass Care Navigators' Hospice Team, you will provide care and support to patients and their families who choose comfort-focused, non-curative care. The role focuses on improving comfort and quality of life by delivering patient-centered nursing care, often with a one-patient-at-a-time approach. What You'll Do
- Provide direct patient care as part of the hospice team
- Monitor patient status and respond to changes in condition
- Document care and maintain EMR records in accordance with hospice principles What We're Looking For
- Graduated practical nurse program
- Current Kentucky LPN license in good standing
- Minimum 2 years nursing experience
- Licensed driver with insured vehicle
- Skilled in EMR documentation and hospice care principles Compensation & Benefits
