About the Role
Part-Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for Signify Health, delivering comprehensive assessments in members’ homes and helping bridge gaps in care. You’ll join a national network of clinicians dedicated to improving health outcomes and connecting people with the right care.
What You'll Do
- Conduct a ~45-minute in-home visit, including medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry).
- Use an iPad and a streamlined clinical workflow; not required to prescribe medications, order labs, or alter current treatment plans.
