About the Role Hospice Nurse Liaison is Granite VNA's primary ambassador at a local hospital, building trusted partnerships and guiding patients and families through hospice options. This highly visible role focuses on education, advocacy, and ensuring smooth, timely transitions to compassionate end-of-life care. You will collaborate with hospital staff and the Granite VNA hospice team to ensure patient-centered service. What You'll Do
- Building and maintaining strong relationships with physicians, nurses, case managers, social workers, and other key contacts at a local hospital.
- Serving as the on-site ambassador for Granite VNA's hospice program and promoting awareness of our services.
- Guiding referrals by facilitating the hospice intake process and transferring essential referral information from the hospital's EMR to our Intake team.
