Home Health Registered Nurse
CenterWell Home Health is seeking a Home Health Registered Nurse to provide personalized, one-on-one in-home care under the Clinical Manager. You’ll work with physicians and clinicians to manage care plans that support recovery and independence for patients.
What You'll Do
- Provide skilled nursing care to patients in their homes, including medications, wound care, and comprehensive assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients' conditions and report changes to the physician and Clinical Manager.
- Educate patients and families on disease management, medications, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
