About the Role CenterWell Home Health nurse, reporting to the Clinical Manager, provides in-home skilled nursing care and manages individualized care plans to help patients regain independence.
What You'll Do
- Provide skilled nursing care in patients' homes, including medications administration, wound care, and assessments.
- Develop and implement care plans with physicians and healthcare professionals.
- Monitor patient conditions and report changes to physician and Clinical Manager.
- Educate patients and families on disease management, medications, and treatments.
- Maintain accurate patient records and coordinate with other professionals.
- Report progress to physician and Clinical Manager.
- Oversee Home Health Aides and ensure proper documentation.
What We're Looking For
- Med surg, ICU, ER, or acute care experience.
