You will manage care plans in collaboration with patients, families and the healthcare team to support recovery and independence. - Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
