About the Role CenterWell Home Health seeks a Home Health Aide with CNA certification to provide direct patient care in patients' homes under the supervision of an RN and according to the Aide Plan of Care (POC). The HHA will support activities of daily living, nutrition, mobility, and safety, while maintaining infection control and good communication with patients, families, and the care team. What You'll Do
- Provide direct patient care to patients under the direction of the RN and according to the POC.
- Assist with self-administered medications by opening bottle caps, reading labels, verifying doses, and observing administration.
- Accurately measure and document temperature, pulse, and blood pressure; report abnormal findings.
- Help patients maintain hygiene (bathing, grooming, oral care, skin and nail care) and assist with feeding and nutrition observations.
