About the Role Mentor Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. You will join a mission-driven team and create relationships that motivate us all every day. Based in community settings, you will work closely with clinical staff to support therapeutic and behavioral plans and help individuals served live as independently as possible. What You'll Do
- Provide transportation and accompany individuals on outings, medical appointments, and shopping in community settings.
- Assist with daily living skills including meal preparation and medication administration.
- Support development of goals across areas such as community participation, budgeting, exercise, nutrition, and vocational responsibilities.
