Community Resource Coordinator II

What you’ll do

• This in-office role supports community connection activities by linking members to local resources to support their care management journey. You will provide members with known community resources, support the care team in identifying member community support needs, and offer health education as appropriate. You will assist in connecting members to cost-effective community and care resources, coordinate outreach resources available to members, and promote awareness of care services. You will support members with community and care resource inquiries, and help prepare and distribute member-related correspondence and educational materials to facilitate successful community connections. You will document and maintain community resources to ensure alignment with health plan requirements and standards of practice.
