About the Role Lyra Health is seeking a Clinical Care Navigator to manage crises, provide in-the-moment phone support, ensure client safety, and connect clients to high-quality, evidence-based providers and facilities. This role does not involve in-person therapy; it focuses on helping clients access care entirely by phone and email, with comfort using new technology and a growth mindset. What You'll Do
- Use evidence-based interventions to manage client crises, including brief risk assessment, safety planning, and de-escalation, with the goal of connecting clients to appropriate treatment.
- Identify and overcome barriers to clinical care options using Lyra’s proprietary software and other methods to connect clients to ideal treatment based on needs and preferences.
- Independently manage a caseload of complex, multi-need clients. What We're Looking For
