About the Role The Clinic Coordinator supports central clinic operations to enable efficient and effective patient care. Working from a regional administrative office with occasional clinic travel, they help coordinate patient information, outreach, and system navigation across the care team to meet financial, clinical, and service goals. What You'll Do
- Ensure all needed patient information is available in the EMR prior to visits, including completing chart review and necessary preparations of patient records for the designated care team.
- Review patient charts for items such as last visit notes, consult and surgical notes, imaging, and lab results not yet documented; identify open quality care gaps and HEDIS measures; consider recent hospital discharges.
