About this role
About the Role The Social Worker educates members on improving health outcomes, assesses Social Determinants of Health (SDOH) and community resource needs, participates in process improvement and other pilot programs as they arise, and works with other Oscar teams to ensure exceptional care for our members. You will report into our Case Management Supervisor. This is a remote position open to candidates who reside in Arizona, Florida, and Texas. What You'll Do
- Actively assist in the coordination of care, in person or remote, across inpatient, outpatient, post-acute, ER, home care, etc., while maintaining confidentiality and professionalism.
